HealthCare Partners, a division of DaVita HealthCare Partners, manages and operates medical groups and affiliated physician networks in California, Nevada, Florida, Arizona, and New Mexico. As of September 30, 2014, HealthCare Partners provided integrated care management for approximately 836,000 patients.

As a coordinated care provider, HealthCare Partners aims to provide higher quality care at a much lower cost to patients. A major way that it accomplishes this is through preventative medicine: identifying at-risk patients and recognizing symptoms early on to take care of illness and disease before it develops and becomes more costly to treat. They have also identified predictable diseases that account for major portions of expenses, and have been able to standardize care, improving overall results while lowering costs.

History

1990's

HealthCare Partners Medical Group was formed in 1992 when Los Angeles-based California Primary Physicians, Pasadena-based Huntington Medical Group, and East Los Angeles based Universal Medi-Co joined forces. In 1994 Bay Shores Medical Group in Redondo Beach and in 1997 Memorial Medical Group in Long Beach joined HealthCare Partners. In 1996, Alliance of Private Practice Physicians became the first of HealthCare Partners' network of independent physician associations (IPAs). Unified Physicians of the South Bay followed soon afterward.

2000's

Growth in the 2000s included three staff-model medical groups teaming with HealthCare partners in California: Greater Valley Medical Group in the San Fernando Valley, Harriman Jones Medical Group in the Long Beach area, and Arroyo Seco Medical Group in Pasadena.

In 2006, HealthCare Partners expanded outside of California with JSA Holdings, Inc., including JSA Healthcare (dba JSA Medical Group) and AmeriDrug Pharmacy in Florida and Pinnacle Health System in Nevada (which became HealthCare Partners of Nevada). Also in 2006, to manage their growing number of physicians and patients, HealthCare Partners Medical Group in California implemented Allscripts electronic medical record system.

In 2008, HealthCare Partners independent physician association network acquired Physician Associates of the Greater San Gabriel Valley in Pasadena. In 2009, Northridge Medial Group in the San Fernando Valley joined HealthCare Partners of California, and Fremont Medical Centers joined HealthCare Partners of Nevada.

2010

In 2010, Talbert Medical Group of Orange County was acquired by HealthCare Partners in California. In Nevada, Rainbow Medical Center and Advanced Medical Center joined HealthCare Partners.

2012

In May 2012, dialysis giant DaVita Inc., agreed to buy HealthCare Partners for $4.4 billion.

On September 10, 2012, HealthCare Partners announced that it would be acquiring ABQ Health Partners, New Mexico's largest independently owned physician group.

On September 12, 2012, HealthCare Partners announced plans to acquire Arta Health Network, Arta Western Medical Group, and Western Medical Management, collectively known as Arta. Arta started two Independent Practice Associations (IPAs) in 1995. Both IPAs contract with more than 800 primary care and specialist physicians to provide care throughout Orange County, California.

In November 2012, DaVita Inc., and HealthCare Partners finalized their merger announced in May 2012. The merged entity is known as DaVita HealthCare Partners Inc.

2013

In April 2013, HealthCare Partners and Cigna launched the Accountable Care Program in Southern California to improve health outcomes and lower costs.

In July 2013, Anthem Blue Cross and nine providers in California, including HealthCare Partners, partnered to expand the Enhanced Personal Health Care Coordination initiative.

In September 2013, HealthCare Partners merged with Arizona Integrated Physicians (AIP). AIP is one of Arizona's largest independent physician networks and comprises more than 700 physicians.

In September 2013, HealthCare Partners was named a top-performing medical group in California for the 10th year in a row by the Integrated Healthcare Association (IHA), a statewide leadership group that promotes quality improvement, accountability, and affordability of healthcare.

In December 2013, HealthCare Partners was selected as one of 123 new Accountable Care Organizations (ACOs) in Medicare, providing approximately 1.5 million more Medicare beneficiaries with access to high-quality, coordinated care across the United States.

2014

DaVita HealthCare Partners and SCAN Health Plan expanded their chronic condition special needs plan (C-SNP) for individuals with end stage renal disease (ESRD) into Los Angeles and Orange Counties beginning Jan. 1, 2014.

In April 2014, Independence Blue Cross (Independence) and DaVita HealthCare Partners announced the creation of Tandigm Health, a joint venture based in Philadelphia.

In June 2014, HealthCare Partners announced its participation in the Fall Prevention Clinical Trial, a national effort to reduce the staggering number of fall-related injuries which affect millions of mature adults annually sponsored by the National Institutes of Health (NIH) and the Patient-Centered Outcomes Research Institute (PCORI).

In June 2014, the Accountable Care Organization (ACO) formed by Anthem Blue Cross (Anthem) and HealthCare Partners in California produced $4.7 million in savings for the first six months of 2013 compared to a comparison group.

In July 2014, DaVita HealthCare Partners announced that Kent Thiry, co-chairman and CEO of DaVita HealthCare Partners, will take the role of CEO of HealthCare Partners working in tandem with the Office of the Chief Medical Officer.

In August 2014, HealthCare Partners California and HealthCare Partners Nevada were listed on Becker's 100 Accountable Care Organizations to Know.

In October 2014, HealthCare Partners California entered into an agreement to acquire Colorado Springs Health Partners (CSHP), a multi-specialty medical group with more than 100 clinicians at 11 locations throughout the Pikes Peak region.

In October 2014, DaVita HealthCare Partners and Centura Health announced a new joint venture that will offer a differentiated healthcare delivery model to help optimize healthcare value and lower costs for consumers in Colorado and Kansas.



Structure

HealthCare Partners, LLC

HealthCare Partners, LLC, is the parent company of these healthcare-focused organizations. It manages the administrative systems of its own staff-model medical group and provides central support services to its independent physician association (IPA) network. The organization oversees operations of medical groups in California, Florida and Nevada, as well as The Camden Group, a national consulting firm.

HealthCare Partners Medical Group

HealthCare Partners Medical Group is the Torrance, California-based medical group and independent physician association that cares for more than 600,000 managed care patients through its employed and independently contracted physicians. HealthCare Partners' medical group employs more than 700 primary care and specialty physicians at more than 60 locations. HealthCare Partners is a provider of Medicare Advantage services in California, and serves the Los Angeles, Pasadena/the San Gabriel Valley, South Bay, Long Beach, the San Fernando and Santa Clarita Valleys, and Orange County areas. HealthCare Partners' independent physician association (IPA) model consists of more than 900 primary care physicians, and is supported by 3,000 specialists. The IPA serves the San Fernando Valley, Pasadena, San Gabriel Valley, central Los Angeles, South Bay and Orange County areas.

JSA Healthcare

JSA was founded in 1969 as John Short & Associates, Inc., a hospital consulting firm, and began to provide primary healthcare services in 1985. It has received recognition as an employer from national publications such as Modern Healthcare, and was also recognized for its internal and family medicine practice by a local Florida Medical Business Newspaper. JSA Healthcare became part of HealthCare Partners in 2006.

HealthCare Partners of Nevada

HealthCare Partners of Nevada was founded as Summit Medical Group in 1996 and is a physician-run medical group that offers primary care, specialty care, urgent care, hospital and sub-acute services, and home health It is the largest private medical group in Nevada, and also provides medical management and network management services through its Health Plan Solutions Division, a third-party administrator licensed by the state of Nevada. These Nevada businesses became part of HealthCare Partners in 2006.

The Camden Group

The Camden Group is a medical consulting firm that is a wholly owned subsidiary of HealthCare Partners, LLC. It provides consulting and management services for medical groups, MSOs, hospitals and other health care organizations across the country.

HealthCare Partners Institute for Applied Research and Education

The HealthCare Partners Institute for Applied Research and Education is a non-profit foundation that is controlled by an independent board. The purpose of the institute is to conduct applied research into health care programs with a focus on managed care.

Major Healthcare Programs

Pioneer Accountable Care Organization (ACO)

HealthCare Partners Medical Group, HealthCare Partners of Nevada, and JSA Medical Group (a division of HealthCare Partners) were selected to participate in the Pioneer Accountable Care Organization (ACO) Model, an initiative sponsored by the Centers for Medicare & Medicaid Services (CMS) Innovation Center. The Pioneer ACO Model is designed to encourage the development of ACOs, which are groups of doctors and other healthcare providers who work together to provide high quality care for their patients. Providers who band together through this model will be required to meet quality standards based upon, among other measures, patient outcomes and care coordination among the provider team.

Brookings-Dartmouth Accountable Care Organization Pilot

HealthCare Partners is one of five medical groups nationally to participate in an accountable care organization (ACO) pilot sponsored by the Engelberg Center for Health Care Reform at The Brookings Institution and The Dartmouth Institute for Health Policy and Clinical Practice. This pilot focuses on improving patient outcomes with coordinated care while slowing the escalating costs of healthcare.

Hospitalist Program for Coordinated Care at California Hospital

HealthCare Partners implemented a hospitalist program at California Hospital Medical Center in response to CHMC's challenges to care for a growing population of indigent and uninsured patients. The hospitalists see not only HealthCare Partners patients, but CHMC's uninsured population, bringing more coordination and oversight to these patients' care. Since its inception, the hospitalist program has resulted in outstanding compliance with best care protocols for heart failure and pneumonia, a reduction in heart failure readmission rates, and a reduction in acute care readmission rates for patients over 64, as well as saving the hospital millions of dollars in charity care.

Research Projects at the HealthCare Partners Institute

The HealthCare Partners Institute for Applied Research and Education, a non-profit organization controlled by an independent board, supports and develops research to improve clinical outcomes and patient experience through education, delivery system redesign, and community programs. Current research is in partnership with the University of California at Los Angeles, the University of California at San Francisco, the Oregon Health and Science University, the Indiana University School of Medicine, and in conjunction with California Healthcare Foundation and Lance Armstrong Foundation grants.

Comprehensive Care Centers

In 2009 HealthCare Partners opened its first Comprehensive Care Center (CCC's) in downtown Los Angeles, CA. There, at-risk patients are provided access to round-the-clock medical attention by a dedicated medical team. Care is coordinated by physicians, nurses, and social workers to ensure that specialist appointments are scheduled, medications are taken and family and caregivers get the support they need in dealing with each patient's complex medical and social issues. The program has successfully lowered hospitalizations of patients. (For patients in the high-risk program, hospital use has declined 20 percent over the two years of the program.)

Clinical Trials Program

For over 20 years the HealthCare Partners Clinical Research Center has worked to find new and more effective treatments for a wide variety of health conditions. Currently HealthCare Partners conducts 60 - 70 Phase II clinical studies per year.

Palliative Care

When it comes to terminal illness, the overwhelming majority of patients say that they would much prefer spend their days in the comfort of their own home. HealthCare Partners offers unique in-home options for end-of-life care. According to The Atlanitic Magazine, 40 percent or more of elderly patients die in the hospital, where as only 20 percent of HealthCare Partners Medicare seniors die in a hospital.
